Chief of Military Records
Dear Sir,
I have the Honor to transmit herewith Blank of Military Records filled out as many as my limited means will allow, especially in orders bearing on the Command Recorded in Company order Book which has been haned [sic] in to the government at Harts Island on our muster out of service Which I find are very important in filling out this blank where the original orders are lost, I have given as much as possible the required information but not sufficient to the credit of this Command.
One Blank Filled.
Copy of Muster out Rolls.
Two Recruiting Handbills.
Guidons No. 1, 2, and V 3 and staff and such other information as may give satisfaction
